fa ترویج زبان توانمندساز: استفاده از اصطلاح "بازماندگان سرطان پستان" به جای "مبتلایان به سرطان پستان" Promoting Empowering Language: Using "Breast Cancer Survivors" Instead of "Breast Cancer Patients" تشخیص، درمان، بازتوانی Diagnosis, treatment, rehabilitation نامه به سردبیر Letter to Editor <div class="WordSection1" dir="RTL" style="text-align: justify;">سردبیر محترم؛ مایلم نکته‌ای مهم در حوزه زبان علمی و اخلاق ارتباطی در مقالات مرتبط با سرطان پستان مطرح کنم. در بسیاری از مقالات علمی، از جمله برخی مقالات منتشرشده در شماره‌های اخیر مجله (1, 2)، همچنان از اصطلاح "مبتلایان به سرطان پستان" برای اشاره به افرادی استفاده می‌شود که در مراحل تشخیص، درمان یا پس از درمان قرار دارند. اگرچه این اصطلاح در ادبیات پزشکی رایج است، اما ممکن است نتواند تجربه زیسته، هویت فردی و نیازهای روان‌شناختی این افراد را به‌طور کامل بازتاب دهد. در سال‌های اخیر، مفهوم "بازماندگان سرطان(Cancer Survivors) " در ادبیات علمی و مراقبت‌های حمایتی اهمیت فزاینده‌ای یافته است. مفهوم بازماندگی سرطان به تجربه زندگی فرد از زمان تشخیص سرطان تا پایان عمر اشاره دارد و شامل تمام ابعاد جسمی، روانی، اجتماعی و اقتصادی ناشی از بیماری و درمان آن می‌شود. براساس تعریف ارائه‌ شده توسط National Cancer Institute Office of Cancer Survivorship، بازماندگان سرطان تنها افرادی نیستند که درمان را به پایان رسانده‌اند، بلکه همه افراد مبتلا به سرطان در مراحل مختلف بیماری، از مراحل اولیه تا سرطان پیشرفته و متاستاتیک را در بر می‌گیرد. به علاوه  بازماندگی سرطان علاوه بر بیماران، مراقبان و خانواده‌های آنان را نیز شامل می‌شود. مراقبت در بازماندگان سرطان از زمان تشخیص آغاز شده و با هدف پیشگیری، پایش و مدیریت عوارض کوتاه‌مدت و بلندمدت درمان، ارتقای رفتارهای سالم، هماهنگی خدمات درمانی، حمایت روانی و اجتماعی، و کاهش مشکلات اقتصادی و مالی ادامه می‌یابد. در این میان، پژوهش‌های مرتبط با بازماندگان سرطان نیز بر شناخت و کاهش پیامدهای جسمی، روانی، اجتماعی و اقتصادی سرطان، بهبود کیفیت مراقبت و ارتقای کیفیت زندگی بازماندگان تمرکز دارند (3, 4). مفهوم “بازماندگی” در سرطان پستان، بخش مهمی از مدیریت بالینی بیماران را تشکیل می‌دهد؛ به‌طوری‌که دستورالعمل‌های مراقبتی انجمن انکولوژی بالینی آمریکا (ASCO) و انجمن سرطان آمریکا (ACS) نیز از عنوان “بازماندگان سرطان پستان” برای تدوین پروتکل‌های مراقبتی پس از تشخیص استفاده کرده‌اند که نشان‌دهنده تغییر پارادایم از تمرکز صرف بر درمان فعال به مدیریت بلندمدت سلامت است (5) استفاده از زبان توانمندساز و مبتنی بر احترام به فرد می‌تواند اثر مثبتی بر احساس کنترل، امید، مشارکت در مراقبت‌های پیگیری و کیفیت زندگی افراد مبتلا یا بهبود‌یافته از سرطان داشته باشد. اصطلاح بازماندگان سرطان پستان (Breast Cancer Survivors)  نه تنها مسیر تاب‌آوری و بهبود را نشان می‌دهد، بلکه هویت فردی و تجربه زیسته بیماران را محترم می‌شمارد. انتخاب واژه‌ها تأثیر مستقیم بر خودانگاره، سازگاری روان‌شناختی و سلامت روان افراد دارد. استفاده از واژه بازماندگان می‌تواند حس توانمندی و کنترل فرد بر زندگی خود را تقویت کند (3).</div> <div class="WordSection2" dir="RTL" style="text-align: justify;"></div> <div class="WordSection3" dir="RTL" style="text-align: justify;"></div> <div class="WordSection4" dir="RTL" style="text-align: justify;"></div> <div class="WordSection5" dir="RTL" style="text-align: justify;"></div> <div style="text-align: justify;"><div aria-label="شکستن صفحه" class="cke_pagebreak" contenteditable="false" data-cke-display-name="pagebreak" data-cke-pagebreak="1" style="page-break-after:always" title="شکستن صفحه"></div>با توجه به اهمیت این موضوع، با احترام پیشنهاد می‌دهم که نویسندگان، ویراستاران و مجلات علمی در مقالاتی که به کیفیت زندگی، پیامدهای روانی، مراقبت بلندمدت و دوره پس از درمان می‌پردازند، به جای عبارت "مبتلایان سرطان پستان"، از "بازماندگان سرطان پستان" استفاده نمایند – مگر در مواردی که تمرکز مقاله صرفاً بر روی جنبه‌های تهاجمی و درمانی بیماری در مرحله فعال باشد.این تغییر زبانی، هرچند کوچک به نظر برسد، می‌تواند در ارتقای حساسیت اخلاقی، بهبود ارتباط علمی با بیماران و احترام به تجربه زیسته آن‌ها نقش مؤثری ایفا کند.</div> <div>از توجه شما بسیار سپاسگزارم و امیدوارم این پیشنهاد بتواند به بهبود ادبیات علمی در مطالعات سرطان پستان کمک نماید.</div> <div class="WordSection1" style="text-align: justify;">Dear Editor, I would like to raise an important point regarding scientific language and ethical communication in breast cancer–related publications. In many scientific articles, including some published in recent issues of the journal (1, 2), the term “breast cancer patients” continues to be used to refer to individuals undergoing diagnosis, treatment, or post-treatment follow-up. Although this terminology is widely used in the medical literature, it may not fully capture these individuals’ lived experiences, personal identities, and psychosocial needs. In recent years, the concept of “cancer survivors” has gained increasing prominence in the scientific literature and supportive care practice. Cancer survivorship refers to an individual's experience from the time of diagnosis through the remainder of life and encompasses the physical, psychological, social, and economic consequences of cancer and its treatment. According to the definition provided by the National Cancer Institute’s Office of Cancer Survivorship, cancer survivors are not limited to individuals who have completed treatment; rather, the term includes all people living with cancer across the disease continuum, from early-stage disease to advanced and metastatic cancer. Furthermore, the concept of survivorship extends beyond affected individuals to include their caregivers and family members. Survivorship care begins at diagnosis and continues throughout the cancer trajectory, aiming to prevent, monitor, and manage treatment-related effects; promote healthy behaviors; coordinate healthcare services; provide psychosocial support; and address financial challenges. Similarly, survivorship research focuses on understanding and mitigating the physical, psychological, social, and economic consequences of cancer, improving the quality of care, and enhancing survivors’ quality of life (3, 4). In breast cancer, survivorship has become a fundamental component of contemporary clinical management. Notably, survivorship care guidelines developed by the American Society of Clinical Oncology (ASCO) and the American Cancer Society (ACS) use the term “breast cancer survivors” when outlining care recommendations following diagnosis. This reflects a paradigm shift from an exclusive focus on active treatment toward the long-term management of health and well-being across the cancer continuum (5).</div> <div style="text-align: justify;"></div> <div class="WordSection2" dir="RTL" style="text-align: justify;"></div> <div style="text-align: justify;"></div> <div class="WordSection3" style="text-align: justify;"></div> <div style="text-align: justify;"><div aria-label="Page Break" class="cke_pagebreak" contenteditable="false" data-cke-display-name="pagebreak" data-cke-pagebreak="1" style="page-break-after:always" title="Page Break"></div>Given the importance of this issue, I respectfully suggest that authors, editors, and scientific journals consider using the term “breast cancer survivors” rather than “breast cancer patients” in studies addressing quality of life, psychosocial outcomes, long-term care, and survivorship-related issues, unless the primary focus of the article is specifically on active treatment or acute disease management. Although this linguistic change may appear subtle, it has the potential to enhance ethical sensitivity, foster more person-centered scientific communication, and acknowledge the lived experiences of those affected by breast cancer. Thank you for your attention to this matter.
