fa پیش‌بینی وضعیت سیستم ایمنی بر اساس تیپ شخصیتی D با میانجی‌گری مکانیسم‌های دفاعی در بیماران مبتلا به سرطان پستان Predicting the status of the immune system based on personality type D mediated by defense mechanisms in patients with breast cancer پژوهشي Research چکیده مقدمه: سرطان پستان، به عنوان شایعترین سرطان، علت مرگ و میر میلیونها نفر در سراسر جهان است. با توجه به اینکه وضعیت روان شناختی می تواند با تحت تاثیر قرار دادن سیستم ایمنی در پاسخ دهی فرد مبتلا به درمان موثر باشد، پژوهش حاضر با هدف ارائه مدل جهت پیش بینی وضعیت سیستم ایمنی بر اساس تیپ شخصیتیD با میانجی گری مکانیسمهای دفاعی در بیماران مبتلا به سرطان پستان انجام شد. روش بررسی: جامعه آماری پژوهش حاضر شامل تمامی زنان مبتلا به سرطان پستان در شهر تهران و گیلان در سال 1398 بودند که از بین آنها تعداد 250 نفر با روش نمونه گیری هدفمند انتخاب شدند. دادهها با استفاده از فهرست وارسی علایم هشدار دهنده نقص سیستم ایمنی، پرسشنامه مکانیسم های دفاعی و پرسشنامه تیپ شخصیتی D جمع آوری شد. برای تحلیل داده ها از روش های ضریب همبستگی پیرسون و مدل معادلات ساختاری با بهره گیری از نرم افزار AMOS و SPSS نسخه 22 استفاده شد. یافتهها: یافته ها نشان داد، بین وضعیت سیستم ایمنی و مکانیسم دفاعی روان رنجور، مکانیسم دفاعی رشد نیافته و تیپ شخصیتی D همبستگی معنادار و مستقیم و بین وضعیت سیستم ایمنی و مکانیسم دفاعی رشد یافته همبستگی معنادار و معکوس وجود دارد. همچنین مکانیسمهای دفاعی در رابطه بین تیپ شخصیتی D و وضعیت سیستم ایمنی در بیماران مبتلا به سرطان پستان دارای نقش میانجی دارد. نتیجه گیری: بر این اساس، در بیماران مبتلا به سرطان پستان، وضعیت سیستم ایمنی از طریق مکانیسم های دفاعی و تیپ شخصیتی D قابل پیش بینی است. بدین صورت که هر چقدر فرد از مکانیسم دفاعی رشدنیافته تر استفاده می کند سیستم ایمنی ضعیفتر داشته و استفاده از مکانیسم دفاعی رشد یافته با سیستم ایمنی قوی تر همراه است. بر اساس یافته های به دست آمده می توان گفت طراحی مداخلات روان شناختی مبتنی بر اصلاح  مکانیسمهای دفاعی و تغییر تیپ شخصیتی D، میتواند به بیمار مبتلا به سرطان پستان جهت مدیریت و درمان بیماری کمک نماید. واژگان کلیدی: سیستم ایمنی، تیپ شخصیتی D، مکانیسم های دفاعی، سرطان پستان   Abstract Introduction: Breast cancer, as the most common cancer, is the cause of death of millions of people around the world. Given that psychological status can be effective in responding to treatment by affecting the immune system, the aim of this study was to present a model for predicting the status of the immune system through personality type D mediated by defense mechanisms in patients with breast cancer. Methods: The statistical population of the present study included all women with breast cancer in Tehran and Gilan in 1398, from which 250 were selected by purposive sampling. Data were collected using a Type D personality, Questionnaire Defense Style Questionnaire and Warning Signs of Immunodeficiency. Pearson correlation coefficient and structural equation modeling were used to analyze the data using AMOS and SPSS software version 22. Results: The results showed that there was a significant and direct correlation between the status of the immune system and the neurotic defense mechanism, undeveloped defense mechanism and personality type D and a significant and inverse correlation between the status of the immune system and the developed defense mechanism. Also, defense mechanisms played a mediating role in the relationship between personality type D and immune system status in breast cancer patients. Conclusion: Accordingly, in patients with breast cancer, the status of the immune system can be predicted through defense mechanisms and personality type D. Therefore, therapists and health professionals can help these people overcome their disease by designing interventions based on modifying defense mechanisms and personality type D. Thus, the more an individual uses the immature defense mechanism, the weaker the immune system is, and the use of the developed defense mechanism is associated with a stronger immune system. Based on the findings, it can be said that designing psychological interventions based on modifying defense mechanisms and changing personality type D, can help a patient with breast cancer to manage and treat the disease.
