مقدمه: سرطان پستان یکی از شایع‌ترین علل مرگ‌ومیر ناشی از سرطان در زنان است و ماموگرافی مؤثرترین روش تشخیص زودهنگام آن محسوب می‌شود. با این حال، مشارکت زنان در برنامه‌های غربالگری همچنان پایین است و نقش عوامل چندسطحی در این زمینه کمتر بررسی شده است. مطالعه حاضر با هدف تبیین عوامل تعیین‌کننده انجام ماموگرافی در زنان جنوب ایران و با تمرکز بر عوامل چندسطحی انجام شد.
روش بررسی: مطالعه حاضر یک پژوهش کیفی با رویکرد تحلیل محتوای قراردادی است. داده‌‌های حاصل از مصاحبه‌ها و بحث‌های گروهی متمرکز با استفاده از این روش تحلیل شدند. سپس برای سازمان‌دهی و تفسیر یافته‌ها از چارچوب نقشه‌نگاری مداخله استفاده شد. داده‌ها از طریق مصاحبه‌های نیمه‌ساختاریافته و بحث‌های گروهی متمرکز با زنان واجد شرایط و ارائه‌دهندگان خدمات سلامت گردآوری و تحلیل شدند.
یافته‌ها: عوامل مؤثر بر انجام ماموگرافی در پنج سطح درون‌فردی، بین‌فردی، سازمانی، اجتماعی و سیاسی شناسایی شدند. در سطح فردی، آگاهی ناکافی، باورهای نادرست، ترس و خودکارآمدی پایین نقش بازدارنده داشتند. در سطح بین‌فردی، کمبود حمایت خانواده و رفتار نامناسب کارکنان سلامت برجسته بود. در سطوح سازمانی و محیطی، محدودیت دسترسی، هزینه‌های بالا و کمبود زمان گزارش شد و در سطوح اجتماعی و سیاسی، ناکارآمدی قوانین، ضعف پوشش بیمه‌ای و چالش‌های اقتصادی بر انجام ماموگرافی تأثیرگذار بودند.
نتیجه‌گیری: یافته‌ها نشان می‌دهد که افزایش مشارکت زنان در ماموگرافی مستلزم اتخاذ رویکردی یکپارچه و چندسطحی در طراحی مداخلات و سیاست‌های سلامت است که فراتر از آموزش فردی، به اصلاح ساختارهای سازمانی و سیاست‌های حمایتی توجه کند.
