مقدمه: درمان سرطان در بسیاری از مواقع با عوارض مختلف همراه است. تبعیت درمان تحت تأثیر عوامل متعدد روانشناختی، جمعیتشناختی، اجتماعی اقتصادی و عوامل مرتبط با بیماری است. مطالعه حاضر با هدف ارزیابی وضعیت تبعیت از درمان و عوامل موثر بر آن در بیماران مبتلا به سرطان پستان در شهر کرمان انجام شد.
روش بررسی: مطالعه حاضر بررسی مقطعی با جامعه هدف بیماران مبتلا به سرطان مراجعه کننده به مطب متخصص انکولوژی و کلینیک جواد الائمه شهر کرمان بود. تعداد 234 بیمار بالای 18 سال به شیوه نمونهگیری آسان وارد مطالعه شدند. از پرسشنامه سه قسمتی شامل خصوصیات دموگرافیک و سابقه درمان، پرسشنامه تبعیت از رژیم درمانی Medication Adherence Rating Scale (MARS) و پرسشنامه عدم قطعیت در بیماری میشل استفاده شد. دادهها با استفاده از SPSS نسخه 26 تحلیل شد؛ از آزمونهای ناپارامتریک و رگرسیون چندمتغیره برای تحلیل استفاده گردید.
یافتهها: میانگین نمره تبعیت دارویی 44/7 (±93/1) بود؛ از مجموع 234 بیمار، 161 نفر (68.8%) تبعیت بالا نشان دادند. میانگین نمره عدم قطعیت 53/85 (±37/18) بود. در تحلیلهای تکمتغیره و چندمتغیره، عدم قطعیت بیشتر (p<0.001) و داشتن درآمد خانواده کمتر از حد کفاف (p=0.017) با تبعیت کمتر دارویی مرتبط بودند. عدم قطعیت در بیماری به عنوان مهمترین پیشبینیکننده کاهش تبعیت دارویی شناسایی شد.
نتیجهگیری: با توجه به نتایج مطالعه، تبعیت از درمان دارویی در اغلب بیماران مبتلا به سرطان پستان در سطح بالایی بود. با این حال، سطح بالاتر عدم قطعیت در بیماری و درآمد ناکافی بهطور معناداری با کاهش تبعیت از درمان همراه بود. این یافتهها نشان میدهد که مداخلات آموزشی و حمایتی باید بر کاهش عدم قطعیت بیماران و حمایت از گروههای دارای محرومیت اقتصادی متمرکز شود.
