مقدمه: سرطان پستان شایعترین بدخیمی در میان زنان است که میزان بقای بیماران با توجه به پیشرفتهای اخیر، رو به افزایش است. از اینرو، ارزیابی جامع کیفیت زندگی (QoL) در این جمعیت اهمیت ویژهای دارد. هدف از این مطالعه، ارزیابی درازمدت کیفیت زندگی، اضطراب، افسردگی و امید در بهبودیافتگان سرطان پستان (BCS) است.
روشها: این مطالعه بر روی 1250 نفر از بهبودیافتگان سرطان پستان مراجعه کننده به یکی از پنج مرکز پزشکی ارجاعی واقع در تهران، ایران، با روش تمام شماری انجام شد. مبتلایانی که درمان خود را تکمیل کرده بودند، بعد از تکمیل فرم رضایتنامه آگاهانه، در سه مقطع زمانی (بلافاصله، یک سال و پنج سال پس از درمان) فرمهای مشخصات جمعیتشناختی و بالینی، پرسشنامه های SF-36، اضطراب و افسردگی HADS، امید بزرگسالان (AHS) را تکمیل کردند. دادهها با استفاده از نرمافزار SPSS نسخه ۲5 و روش تجزیه و تحلیل مدل ترکیبی تجزیه و تحلیل شدند.
نتایج: در مجموع ، ۱۱۱۵ نفر از بهبودیافتگان تمایل به شرکت در این مطالعه داشتند. از این تعداد، ۸۵۶ فرد (۷۶.۷٪) مطالعه را تا آخرین مرحله پیگیری، به اتمام رسانیدند. در تمام خرده مقیاسهای SF-36، شامل مؤلفههای جسمی ((PCS و مؤلفههای روانیMCS) ) ، بهبود مشاهده شد. تجزیه و تحلیل مدل ترکیبی نشان داد که اضطراب با PCS و MCS ارتباط معکوس دارد(P=0.01) . سن بالاتر در زمان تشخیص با کاهش عملکرد فیزیکی همراه بود ) P<0.001( . برعکس، امید و تحصیلات عالی ارتباط مثبت معنیداری با کیفیت زندگی نشان دادند. (P<0.001 و P=0.01 (
نتیجهگیری: مؤلفههای جسمی و روانی کیفیت زندگی در بهبودیافتگان سرطان پستان طی پنج سال پس از درمان، بهبود یافته است و عواملی مانند اضطراب و امید، بر کیفیت زندگی تأثیرگذار بوده است. بنابراین همان گونه که برنامه بیماریابی در تشخیص به هنگام سرطان پستان مفید بوده در مورد انجام به هنگام برنامههای غربالگری منظم در مورداضطراب و افسردگی در کلینیکهای پیگیری سرطان پستان بهتراست اقدامات اولیه صورت گیرد.
